Position Overview:

Our Chronic Care Nurses are here to provide that support. They are the trusted connection between patients and their care teams, offering guidance in times of need—whether that's managing medication side effects, helping with transportation, or advising on when to seek urgent care. In this role, you'll make a real difference by offering consistent, compassionate care to chronically ill patients, ensuring they're never alone when it comes to managing their health.
Key Responsibilities:
Conduct regular check-ins with patients to discuss their condition, follow up on appointments, and implement new instructions from their healthcare providers.
Manage a caseload of 200-250 chronically ill patients, providing them with ongoing support and care coordination.
Monitor patient progress and identify when additional communication or intervention is needed to address concerns.
Build strong relationships with patients by understanding their unique challenges and providing empathetic care.
Establish trust with new patients and their families, guiding them through their healthcare journey.
Serve as a vital communication link within the patient's care team, collaborating with providers and office staff to ensure continuity of care.
Help patients prioritize their health by explaining their conditions and treatment plans in a clear, compassionate way.
Act as a patient advocate, addressing medical questions and concerns promptly.
Use technology and systems to track patient progress, communicate with healthcare teams, and manage care.
Ensure the accuracy of patient records, utilizing Electronic Medical Record (EMR) systems to document actions taken and communicate with team members.
Qualifications:
Active, unrestricted LVN, LPN, or RN license required.
Graduate of an accredited LVN, LPN, or RN program required.
Minimum of 4 years of clinical experience required.
At least 2 years of experience in home health, primary care, or long-term care preferred.
Ability to work full-time, Monday through Friday, with shifts available from 8 a.m. to 4:30 p.m. EST or 11 a.m. to 7:30 p.m. EST (or 8 a.m. to 4:30 p.m. PST).
Minimum internet speed of 12 Mbps upload and 20 Mbps download required.
Comfortable working in a home environment while adhering to HIPAA compliance standards.
Proficiency in using Electronic Medical Records (EMR) systems.
Excellent verbal and written communication skills.
Strong organizational skills with attention to detail.
Ability to manage multiple priorities effectively in a fast-paced environment.
